New Patient Registration

Plan to arrive 15 minutes before your first appointment to allow time to register as a new patient.Upon arrival, please check in at the front desk with your patient forms and insurance information. You will also be asked to review and sign a Healthcare Proxy and HIPPA forms.

Once registration is complete, you'll be escorted to an exam room. One of our spine specialists will review medical history and perform a physical examination. After your examination and consultation, you will receive an explanation of your condition with instructions on necessary follow-up diagnostics, appointments and potential treatment options.

New Patient Forms

To help expedite your first appointment registration, Comprehensive Spine Care recommends that you print and complete the new patient forms below before you arrive for your first appointment.

Please Note: If your condition is the result of a recent work injury, please refer to our Work Injured Patient Page for more information about appointments and patient forms related to work injuries.  If you have questions about whether or not your spine problem qualifies for Worker's Compensation, please contact our office at 314-991-7707, and we'll ask you a series of questions to help determine if your condition qualifies.

Information Required For Your First Appointment

Please bring your completed new patient forms along with all of the applicable items listed below. 

  1. Referral(s) and pre-authorization(s)
  2. Previous x-rays, MRI scans, or CT scans: films and reports must be provided
  3. Medical records of previous testing or treatment (EMGs, physical therapy, etc.)
  4. Any brace or support that you are currently or have recently used
  5. Insurance card and applicable co-payment 

Please note, if we have not received your relevant medical records and films at the time of your scheduled appointment, your appointment may need to be rescheduled.

Scheduling Follow-up Appointments

Follow-up Appointments
If the doctor requests a follow-up appointment, please try to schedule your next appointment before leaving our office to insure that your follow-up appointment time is available.

Post-operative Appointments
When you arrive home, please call our office to arrange for your post-operative follow-up appointment.

Our office is open Monday through Friday between the hours of 8:00 AM and 4:30 PM. The answering service takes calls 1 hour during lunch and before and after office hours.
